Renin-Angiotensin-Aldosterone System (RAAS) is a major long-term regulator of blood pressure and Na+ and K+ balance. Each nephron releases renin when it receives various stimuli, either as a fall in artery pressure or as a decrease in distal tubular sodium supply. Renin liberates angiotensin-I from angiotensinogen. Angiotensin-I is transformed to angiotensin-II largely in pulmonary circulation by angiotensin converting enzyme (ACE). Angiotensin-II raises blood pressure by direct arteriolar vasoconstrction, promotes sodium retension, and stimulates the secretion of aldosterone by the adrenal cortex. Aldosterone also exerts effects to restore sodium balance and lift arterial pressure. In addition to the effects that are linked to blood pressure and electrolyte homeostasis, Angiotensin-II also directly promotes inflammation and therefore, development of atherosclerosis. Determination of circulating Ang-II, renin, and aldosterone can aid in the study of Primary Aldosteronism and in the study of other forms of hypertension.
